Positive Solutions Behavior Group is a family-focused resource that continuously strives to increase opportunities for individuals with autism spectrum disorder and other developmental needs by applying the principles of Applied Behavior Analysis (ABA) in the community, home, schools, and clinical setting. We recently expanded our operations to include Targeted Case Management and are seeking a full-time Targeted Case Manager to provide services in the Northern Kentucky area to recipients seeking mental health services.

-Provide team leadership.
-Plan supervision schedules and provide consultations to team.
-Responsible for oversight of all on-going paperwork and documentation for program.
-Assess, plan, implement, monitor, and evaluate client’s health and needs.
-Train, plan, and assign case assignments.
-Handle case assignments of up to 25 clients.
-Create and implement treatment plans.
-Help clients make informed decisions and advocate for client.
-Provide education and support to team and clients.
-Gather and make appropriate referrals to client.
-Prepare and maintain client records and notes in data collection system.
-Attend necessary trainings and certification courses.
-Participate in networking activities and related community organizations.
-Continue to grow and market the Targeted Case Management program.
-Assist in billing needs.
-Other duties as assigned.
-Full-time with local travel

-Bachelor’s Degree in a human services field.
-Targeted Case Management Certifications or the ability to complete Targeted Case Management Certifications within 1 month of hire date.
-Excellent interpersonal skills and ability to work effectively with team.
-Ability to communicate effectively, both orally and in writing.
-Valid CPR Certification or the ability to complete CPR Certification within 1-2 months of hire date.

Preferred
-3 years experience in Targeted Case Management services.
