Targeted Case Managers provides services and support to recipients diagnosed with:
- Severe Emotional Disability (SED)
- Serious Mental Illness (SMI)
- Substance Use Disorder (SUD)
- Co-occuring Complex or Chronic Physical Health Condition (CCPHI)
